Digital Twins for Chronic Disease Monitoring and Personalized Healthcare

Overview

This workshop will explore healthcare digital twins for continuous monitoring, early detection, and personalized management of chronic diseases. Conditions such as colorectal cancer, inflammatory bowel disease, and chronic kidney disease affect millions of individuals worldwide and impose substantial clinical and economic burdens, yet current care pathways rely primarily on episodic clinical encounters, laboratory testing, and reactive interventions. While these approaches are clinically validated, healthcare providers often face substantial workload pressures and limited time per patient. In addition, reliance on in-person visits and diagnostic testing can be time-consuming and costly for both patients and healthcare systems. A key research challenge is how to move from episodic and reactive care toward continuous, proactive, and personalized care.

Recent advances in sensing technologies, health data infrastructure, and artificial intelligence have created opportunities to address this challenge. One promising approach is the development of healthcare digital twins, computational representations of physiological systems that integrate multimodal patient data with computational models to support continuous monitoring and personalized disease management.

The workshop will feature invited talks alongside contributed presentations selected through an open call.

Scope and Topics of Interest

The workshop will focus on several thematic areas relevant to healthcare digital twins.

Multimodal Data for Digital Twins

Integration and analysis of diverse health data sources, including:
  • physiological sensing data from wearable devices
  • smartphone-based data collection and mobile health applications
  • behavioral and lifestyle monitoring
  • electronic health records and laboratory data
  • patient-reported health information

AI and Computational Models for Digital Twins

Computational approaches for modeling physiological systems, including:
  • mechanistic and physiology-based models
  • machine learning and data-driven predictive models
  • hybrid models combining mechanistic and AI-based approaches

Disease Progression Detection and Personalized Management

Digital twin–based approaches for improving disease monitoring and care strategies, including:
  • early detection of disease progression
  • population-level predictive models
  • AI-assisted disease assessment and decision support
  • personalized disease management strategies

Healthcare Systems and Access

Integration of digital twin technologies into healthcare practice and delivery systems, including:
  • clinical integration of digital twin tools
  • applications in primary care and preventive care
  • improving healthcare accessibility and continuity of care

The workshop invites one-page abstracts presenting research, emerging ideas, case studies, prototypes, or work in progress related to healthcare digital twins, chronic disease monitoring, and personalized healthcare.
